Most BBs I encounter on old, used bikes only come out with the help of a bench vice. I clamp the BB tool in the vice facing upwards, lower the frame onto it, and then turn the frame by applying force to the places where tubes intersect (holding both ends of the top tube works well).
If you have no vice then you might need a long spanner and will have to jump on it. But for gods sake check the threading here: http://sheldonbrown.com/cribsheet-bottombrackets.html. You don't want to tighten it further!
You do indeed need 116mm. Velobase is your friend. And make sure it's JIS or it'll sit way too far in-board.

I have something you may like, it's a 58 x 58cm c-c smooth welded Fanciulli frame, probably early nineties but maybe late eighties? Not sure what particular Super Vitus tubing it is, but they were equivalent to the Columbus SL/SLX tubings in terms of material thicknesses. Complete with chrome Columbus forks.
I rode this bike across London in its current build state, and it's really nice and responsive. 130mm rear spacing and internal cable routing. I bought it for the Campag parts and I don't need the frame (too small for me).

Fanciulli was a Bikeshop in Basel, Switzerland. Its not existing anymore. They had their own bike collection manufactured in Italy.I know they built some frames in Olmo tubing as well, and I found this bad boy on Velospace build with SLX.
