one obvious point they seemed to miss in the bike vs. stationary lorry scenario (not sure if this has been said already) was with the risk factor of "passing on either side and stopping in front of the lorry" the risk factor of stopping in front of a stationary lorry totally depends on how far in front you are and if the driver can see you. not knowing if the surveyor assumes that the driver can see the cyclist or not makes an accurate level of risk difficult to judge and the question difficult answer.
obviously overtaking the lorry in the first place is a risk in its self, that risk being much higher if you pass on the nearside. -

8:20ish this fine morning on old street I saw a chap with a white skate lid riding a bright yellow ss conversion with fluro yellow saddle, yellow v brakes(!), yellow grips on riser bars and deep section 26" wheels. looked like a blast to ride!
then a few mins later on Shaftsbury ave another white skate lid wearer this time riding a silver BMW gangsta with Paul royal flush cranks and a black aerospoke up front. mate your chain was so slack that I could hear it over the traffic! I thought it was gonna fall off right there and then, dangerous stuff.
seriously though, if you work in the west end and need a chain breaker to sort that shit out just let me know and I'll hook you up :) 2+ grands worth of rig deserves it! -
wow. its enough to make you build your own stupidly big comedy weapon and costume just so you could smack him over the head with it in some pseudo historical battle on Hampstead heath giving him just enough time between whacks to get little lugs on his asthma inhaler and eventually being pulled off by his mum who is dressed as the Empress of Fantasia ...(shudder)...
