Does anyone run this configuration, and are there ensuing problems with chain adjustment in conjunction with fore-and-aft positioning of caliper mountings with the brake disc being so mobile?

Tilt bike till grip is in vertical plane. Poke park tools 2.5 mm hex key deep into grip with a dribble of water and work it off with a big wriggle and more water between hex key and grip. Replace it with a copious application of 'shock waves' hair spray when your job's done. Easoid peasoid.

Is the thread really gone, or just a bit flat? Your local fixit shop might get enough thread back with the big thread-chaser to set the lock-ring in with a bit of locking potion. You would have to get it totally reamed to use a threadless. I've used a Royce unit for several years without it working loose, but it was a bit spendy.

I agree with Haj- clean and degrease,then start ' loctite-ing ' . Also, if it's going to be a fairly permanent set-up, the other famous 'ite', Araldite, will stop most things unscrewing, and still let go with a good warm-up if you really need to get it off again.

Julietty, you can fit a harry but you need to set about the inside of the clamp with a 10 or 13 mm parallel round file. Using your skill and judgement, start whittling away at the two obvious contact points which are preventing clamp closure, then generally enlarge the radius slightly. You would probably have to take a radgie on it to render it useless, and after all, they only cost a fiver. I might have had to fit a 10mm longer clamp bolt to span the gap, but it's on and working.
