d - lock to the head is a bit ott, could have killed him with a decent strike. but that chinesey dude started it and u shouldnt start something u aint prepared to finish. could of been much worse for him imo.

Start off at 1mm and work your way up. It's not hard.
flawed, imo
i would drill straight through with the 6, small drills can wander especially when going through the other side. and way less dickin around.
but yes its a simple job that u can do urself at home.

yo tim, u didnt mention how far it's out of alignment (at least i didnt see it anywhere) but imo if it is <5mm putting a washer/spacer on the hub to push out the rear sprocket is the best/cheapest option.
larger misalignment than that i would go for a longer bb rather than a shorter one and switch the chain ring over on the spider. -
penetrating oil, plus gas is a goody, or wickes do their own brand of it.
remove as much crap as you can before spraying it on and leave overnight.
works for me.
wd-40 is not penetrating oil.

ive got one, the shank has been turned down to 13mm so it will fit in a standard chuck. what area u in? im central now nw later.
