yeah, you need cross top levers for that, easily fitted and not much £££

What do the two inner cables attach to though as the caliper cable clamp has room only for one cable?

it only uses one cable, you splice the outer and the cross top lever goes between the two with the inner cable running through it. it works slightly differently to a normal lever.
normal levers pull the inner cable and the outer stays static, where as the cross top lever pulls the outer and the inner cable stays static. both result in closing the brake caliper.

Is it possible to operate the front brake from two separate positions one lever on the tops and another one on the drops?
