Did you lose some spacers when the crank fell off?

Probably lost spacers and maybe a bearing shield.
But the crank axle interface is likely to be a bit fucked now.

Bearing shield was still there but yes I think a spacer has gone. Am I correct in saying that there is no definitive answer in the number or thickness of spacers that needs to go into this system and it is very dependent on the frame?

In the past I have also had the preload bolt on the 24mm version of this crank fall out and my fix for that was to mitre bond it back in which is a 2 part glue.

Yes
So I was riding this morning an my FSA non drive side crank arm fell off. It is BB30 gossamer with wavy washer on non drive side.
Does anyone have any experience of this happening? When I reattach the crank arm it 'bottoms out' before I can get up to recommended torque and does not compress the wavy washer as it should although the crank does go tight without binding.
In my opinion I need some spacers to take up the gap between the back of the crank arm and the wavy washer. Anyone have any advice?
