Also, May not be end of the world:

Drill it out?

Link doesn't work but if say the better question was how do you safely install one in a carbon steerer in the first place?

Why do you want to remove it? I assume you don't want to put any more pressure on it, but can you not just install a bung above it?

Maybe that works??
In any case, according to the document, Cannondale designed this star nut and topcap to work together. The start nut is at the required 40mm depth. It actually says specifically NOT to remove the star nut and use an expander.
Up until 2007 Cannondale used the star nut as standard in their carbon steerers.
I'm not going to punch it further down the tube - that will def. ruin the fork.
After reading about it I'm tempted to leave it alone as I have the correct sleeved top cap.
I might also keep an eye out for a new fork. -

Because initially I was worried that it was an incorrectly installed star nut. After some research though it all appears to within spec of a 2006 Cannondale Slice Premium fork.
I still can't be certain though that it was pressed into it's current location with the correct tool, hence the doubt.

Is it moving? If not then it is probably correctly installed and not like to cause a failure, however trying to remove it may damage the fork and cause it to fail.
How does one remove a star nut from a carbon steerer? It's an old cannondale slice fork so I'm assuming it was factory installed. I'm not keen on keeping it in if I don't have to though. Will replace with expander bung.
But really don't want to do any damage, I value my teeth.
