Does the chain become slack first? Check it after a week to see if it needs retensioning, it might be the wheel creeping forward in the drop out.
It could be a worn chain though, how old is it?
Has it always done this or just started recently? -

If the chain is tight enough initially then it's probably that the nuts needs to be a bit tighter.
I think the anti turn washers are unrelated to this issue, but I had yellow and brown for forward facing dropouts.

That makes sense, in effect I'm putting extra pull into the chain whenever I stop which must be working the axle forward.
Probably not much I can do other than tighten the nuts as much as I can and retension the chain every week. It kind of defeats the purpose of a low maintenance city bike though :(
I've got a Shimano 8 speed hub with a coaster brake. I'm finding the chain works loose and it jumps off every couple of weeks at least.
I can't see why it comes loose so fast. Is it just that I'm too girly to tighten the nuts properly (I'm using a foot long wrench for leverage) or is there some trick to getting the chain tight I am missing?
The bike has horizontal fwd facing dropouts not track dropouts, and I'm using the black/grey anti-rotation nuts.
