I have valve extenders on and have removed the valve screw

Could gravel have got into the rim during that tube change? It's possible if your rim tape is knackered or it got in through the valve drilling.

I had a similar issue, drove me nuts. Just needed to tighten the inner nut/cone.
Might be the same with you.
I just transported my bike to London and after a couple of days of dodging traffic my back wheel is starting to rattle more and more. The wheel itself is firmly on. There is no sideways motion and it rides true yet it sounds as though it is loose. Any suggestions?
The wheel is only a month and I haven't removed it since first putting it on though I changed the inner a week ago.
