I suffer from 'bike horn'.
I blame the vibration of bumpy roads stimulating my perineum.
It plays havoc with my lycra, II can tell you.

My mate and I bought these for a laugh about 3 years ago because the Evans on the corner near my work was selling them off cheap. I can confirm that they aren't just a toy, they really are F***ING LOUD!!!!
I haven't used mine because my bike has no lugs, so I've got no bottle carrier (didn't even realise this at the time I bought it) but he says it's really effective. Especially on the paying-no-attention peds.
It also works on twats who think it's funny to blast their horn at cyclists. Just catch up at the next light, ride up on the outside and let the driver have it, straight in the right ear through his open window. ;-)

Not used it yet. Maybe tonight on the way back home I'm thinking High Street Ken. TOOOOOOOOOOOOOOT!
It mounts with velcro so no bottle mount necessary.

Anyone seen this silly thing?
Maybe of interest to those based in London?
