being a 'professional' mechanic i should know what these are called but can't remember... all bike shops should have them, off the top of my head they're about 20p for plastic ones and £1 for nicer alloy ones (that won't pull through the 'oles no matter how hard you mash the brakes); try asking for some 'little drop lever cable end grommet widget fuckers' in your lbs, if they haven't a clue just explain the problem; 10/10 for diagram effort though

Look for bicycle brake cable ferrules.
Google is your friend -

From your drawing it looks like you need the Weinmann types.
They don't list them on their website, but Freeman's Cycles have them.

Just use the ones you have and a common or garden small washer.
I have a problem with some drop-bar brake levers. Essentially there is nothing to stop the shrouds of the brake cables from passing through the holes which the wires must pass through so that the barrels can engage with the levers. This means when you pull the lever it just pulls the whole cable-outer through the hole too. (I have metal endcaps on the cables) It looks like there must be some kind of flanges that fit into these holes to stop the shrouds passing through whilst allowing the inner wires through. Maybe something like this:
Does anybody know what might do the trick, can you buy these flanges or whatever they would be? I don't really want to have to machine them myself. The levers are Dia Compe.
