What frame is this? Rare low profile

It wouldnt surprise me if this is turns out to be a dubble post but since I have no clue what it's name/brand/model is, I can't search for it.

Anyway, what I'm looking for is a frame (eh... fork?) where the handlebar is attached to the crown of the fork. I've seen this once in a picture with bullhorns.

Does this type have a name? or is it just one brand/model? In worst case could it been a custom made one?

Oh and btw, I know I'm no good at explaining so took some random racing bike picture of google and modded it in paint, hope it will help.

It is much further forward than being on the drops. Drop bars don't end up with the drops over the front half of the wheel. It would be like using drop bars with a 30cm stem I would think.
