Buy a decent stem and torque the bolts the correct amount using a torque wrench. I'm guessing it was just a shitty stem made from shitty metal...

Totally agree. Just buy a new one.
On-one do loads of sizes and are of decent quality.

Thanks, pretty much what I expected.
Having just had a quick google on torque wrenches they do seem to be eyewateringly expensive. I usually guestimate but this time the difference between not enough and too much was so slim.

The torque settings are to avoid too much force on the bars, as opposed to protecting the bolts/threading (I think). So the bolts should have taken the extra (if there was any) torque. If you were using a standard little alle key then it is difficult over torque the bolts by very much (providing you use the tool in a normal manner). One trick is to have various lenghs of tools for different parts of the bike, which require different torque. This looks like a good buy (might buy it when I start assemblying next project with carbon bits). BBB torquefix £41

I've been adjusting the set up on my OTP Lager and tried flipping the stem to lower the bars a bit. To complete this I had to remove the bars and put them back in the right way up.
All fine so far.
On the way into work this morning I hoped up a low kerb and the impact of landing made the bars move a bit. When i got into work i gave the bolts a tighten with a small allen key and POP! with what I thought was very little load the bolt stripped the thread out of the stem.
So my question is: What did I do wrong and what can I do to fix it. I think the solution will probably be "buy a new stem and get someone who knows what they're doing to fit it", but that way I won't learn anything.
