Save as 2-colour uncompressed image, white engraving on black roundel as you've shown (.png is probably best) at the original 1350px square as that scales properly to the engravable area at 1200dpi. Upload as an attachment here and I'll grab it. They'll have to vectorise it to create a tool path for the rotary engraving machine, but it's probably best to give them the raster image and let them handle the vectorisation.
