Get drunk and hammer an Allen key into what remained of the bolt.
I think the rum made me less worried about fucking up the shoe.
This works well with ATAC cleats that have been walked on and the allen key hole gets crushed. If the screw is stuck fast I've always just sawed through the cleat and bolt then used a flat head screwdriver to get it out.
