You can put what you want on the birth certificate I believe, a colleague of mine was looking into this as she is still married to her ex, and pregnant with the child of her new boyfriend, she didn't wish to name said sprog with her ex-husbands name, hence the inquiry.
I'd simply call the child [First name] Van [last name].
Try it- an example would be Bothwell Van Buckfast.
Lends an air of glamour to the most quotidian of names.
