As the Surly Fixxer thread piece seems to only slot onto the splines for a freehub body, I don't see how that piece could be made to work with a wheel with a freewheel thread on.
I just bought one. appaz it'll give a chainlin of 46mm on a track frame with a 7 speed threaded carbon wheel respaced to fit centre. This is because its designed for geared frames. I plan on fixing this by adjusting the converter some how, but I'll need to have a look at it first to decide if that is possible, if not flipping the sprocket will get rid of 4mm or so offset I think.
I will attach it to the wheel with pleanty of torq and grub screws through the hub and adapter.
I have found axles from miche hubs most suited to the job. the axles are threaded most of the way and the bearings sit on screw on bits.
