They also come with the 3.5mm allen key to secure the brake cable
Either Chorus or Record but a very wise man (you) told me to check the cable clamp...

And it's 100% a 1993/1994 Chorus Rear Derailleur RD-11CH, 8speed, maximum sprocket 26T, total capacity 26T
The derailleur also had the same look in 1995 but had 'Chorus' printed below Campagnolo.
