I'm confident that riding fixed has been kinder on my knees than riding geared. I used to punish my knees by riding too high a gear on my commute but that all changed at the age of about 30 when I ditched gears for the commute and (because I lived on the top of Forest Hill) developed a love of a fairly low gear, mostly 67 to 70 inches or so.
Sixteen years later my knees are still happy. I still use gears for some rides, and have to resist the temptation to whack the bike into 53x11 just because it's there.
