The cable stops in the frame come out (just screw in a long wood screw to the hole and use pliers to remove them) then you have a much bigger hole to aim for.
You don't need the liner for the brake cable (as it crosses the frame) and a little piece of standard cable liner is all that is needed for the gear cable where they exit the frame before the bottom bracket. This saves .066 grammes.
