It's just an old Schwinn rigid. My polo frame is similar a 400mm fork gives it a 70deg HT. Both frames are of a similar age - 12-15yrs.
I'm sure I can find some forks on ebay or somewhere in the depths of the internet. The frame is just sitting here with knackered dropouts and no cable guides. I won't be able to sell it so I might as well use it.
