It's a standard British threads Shimano UN54.
I have no idea how it could have happened - The non-drive side cup had become recessed into the BB shell, and the drive side extended by 3mm - 4mm and once I removed the cranks, I could remove it by hand.
I thought that there might be stripped threads, but they were all clean, and the BB shell clear of any crap.
