Just tried moving the chainring to the inside of the spider. This gave me something like a 40 mm chainline and didn't reduce the noise coming from the chain at all.
Next thing to try will be trying to adjust the BB position a bit, which maybe will get me closer to 42 mm.
I'm starting to wonder if the chainline is really what's causing the noise though.
