Buy some nice 2nd hand campag road cranks and convert them with single chainring bolts (or find some already converted for sellz), use a 113mm ISO bottom bracket with a 1mm spacer on the drive side (or a 111mm ISO bb with a 2mm spacer... should be alright) this should give you bang on 45mm... campag will look alot nicer IMO (and be superior quality?)
