I've got SPD SL pedals on all of my bikes and tend to go through a lot of cleats. I've never had a problem with them before. I fitted a new set a few weeks ago and two of the yellow rubber pads fell of during the first couple of rides.
I sent a polite and non moany email to Shimano saying that this was the first time I have had a problem and that their cleats are normally more durable than this. Received an email back from Madison within 24 hours with an apology and an offer of a new set of cleats.
Can't say fairer than that.
