is the stem tightened? I mean is the expanding bolt screwed all the way in?
sorry about this question if it sounds stupid to you, but its good to start from the beginning:)
I would suggest obtaining a big spike, bigger than 22.2 in diameter [round in shape, obviously] - like a wine bottle cap or sth and gently tapping it in with a hammer.
pics would help, of both stem and the steerer.
