I (like most people) have a bit of drop on the top of road bars, just before the hoods. The modern trend is away form this. If you look at road bikes form the 90s there is often 30mm+ of drop to the hoods.
When I'm using bullhorns, I set them up just like my hoods. They drop 20mm ish.
I wonder if people moaning about this (thinking it's just a fashion thing) have ever got out of London and ridden up some hills?

They also spin at 90 rpm up the hills.
This didn't used to be the case. I know I don't spin at 90rpm uphill when fixed. I get out the saddle, and like to have dipped bullhorns to hold onto.

I understand where you're coming from miro, but because they do it in the ol' days doesn't necessary mean it's comfortable, after all, cycling only just begun a century ago and we've seen many advanced in not only technology but also ergonomic and bike fitting.
I did experiment with bullhorn sloped slightly downward, it's a little easier riding up big hill, but that's about it, everything else I find it hard to just relaxed and let my hand rest on the hood without feeling like I'm sliding forward.
Why did I put it in that position at the time? because I though it look cool.

The thing is the only 'right' position is the one that the rider finds most comfortable.

It's worth mentioning I do most of my riding outside of London and in the hills.
With bullhorns in hills I'm either;
spinning downhill with hands on the tops of the bars
grinding uphill in the saddle, with hands on the tops
or uphill out of the saddle with hands on the endsThe majority of people riding fixed around (hilly) Bristol use bullhorns or drops/hoods. Lots of them tilt them slightly downwards.
In london most people seem to stick with risers. This makes perfect sense as it's pretty flat.

I hated myself somewhat for putting them on. But find them a million times more useful than the hoodless drops I had.

I suppose with brake hood, you have something to hold due to the hood shape, whether the horn on a bullhorn bar is circular, which may not be as comfortable as a brake hood lever.
I like having brake hood lever as it mean I can grip it better than the bullhorn bar.
