It's worth mentioning I do most of my riding outside of London and in the hills.
With bullhorns in hills I'm either;
spinning downhill with hands on the tops of the bars
grinding uphill in the saddle, with hands on the tops
or uphill out of the saddle with hands on the ends
The majority of people riding fixed around (hilly) Bristol use bullhorns or drops/hoods. Lots of them tilt them slightly downwards.
In london most people seem to stick with risers. This makes perfect sense as it's pretty flat.
