I always look at the threads first, my fixed gear does have stripped threads (not by me) so use one of these although mine is 110mm which they don't seem to have in stock at the moment, works fine and has been in a couple of bikes now, need lots of grease and torque to stop it unscrewing itself!
My most annoying fuck up so far was done whilst changing gear cable on my roadie I managed to jam some cable inside the mechanism (on one of those fancy flappy paddle shimano brifter jobs) I was not best pleased!
Managed to redeem it after much Google-fu (here) and purchasing this (just asked my LBS to order me one)and lots of tea I was able to take it apart and rebuild it, very satisfying but the shifter was out of action for a month!
