While on the subject of tandems, why are the cranksets always installed the same way like in Fig. 1? I'd think that installing them at a 90 degree angle would help overcome the dead spot (Fig. 2). Will the bike become unstable if the two riders' movements aren't synchronised?

People have experimented with different phasing http://en.wikipedia.org/wiki/Tandem_bicycle#Crank_phase but the conventional 0° phasing is the only one which really allows out of the saddle efforts.

Standing up on Swains lane on a tandem is hell on earth. Fast short hills out of the saddle are fine after a few miles but slogs are best done sat down in a silly low gear and sweat it out. Tandems are so much fun it's surprising more don't indulge.

Got some good ideas.
Hadn't really thought it had been done before.....Although it has, and been done well.
