Might want to edit the name of your employer out of there.
These look like very similar questions phrased the opposite ways round. I would go with tickyboxes for the "What stops you?" question and have a write-in for "What would encourage you?". Also the "if no" for the second part could be "what would encourage you to cycle more often?", to catch the part-timers.
