Try a bigger tray? Or a covered litter box? We have a massive one for our two cats, but in the past, when the tray has been smaller, this has happened (one cat not wanting to get too much into the litter, so her ass sticks over the edge of the tray).
(btw, the sick one is still kicking. Although she doesn't walk around or hang out at all any more. Just sleeps under the bed in her comfortable hole with her food and water and a short few steps to the litter. She seems comfortable for now.).
