• If you go for a SA 3-speed you can definitely use a normal downtube shifter (friction, not indexed) rather than a twist-grip. You might be able to use a downtube shifter on a 5-speed hub too.

    I've experimented with a similar thing - I used a double 46/42 chainset, running 46/18 singlespeed, or if I wanted gears running a 3-speed SA hub on the 42mm ring with a 22t sprocket on the hub, which means the chain length didn't need to be changed (teeth add up to the same number). It gave me IIRC 65" high, 50" middle and 35" low, or something around that.

    As for the cable stop, you can indeed get clamp-on ones. I dunno if they're small enough for chainstays though. There isn't much force going through a hub gear cable so you could make a little sheet-steel bracket and fasten it with a small jubilee clip.
