Just put a freewheel, they're a lots gentler on the knees than a fixed gearing that's a bit more spinner.
I'm in similar situation as you, but my doctor's a cyclist which help, he told me to get a freewheel bicycle preferably with more than one speed so I won't have to stand on the pedal to cycle up the hill for instance.
