not really, if your bike is a custom-made that's design to fit you perfectly, there's no need to change stem length/heigh.

I'm a bit worried about the sharp corner at the cable exit. There are better ways of solving the cantilever problem, although disc brakes make the problem go away completely and the only people who should still be using cantilevers are those restricted by the UCI cyclo cross regulations. On that bike, with skinny rims and balloon tyres (a bad idea all by itself, regardless of the brakes), there's a good chance you won't be able to get an inflated tyre past the brake blocks even after you've unhooked the straddle cable.
Oh, and the rear mudguard is so short that you're going to get a wet arse, so it might as well not be there at all.
Epic fail on at least 3 counts.

Not really. If you want to change the stem then just get one of those little hanger thingies that goes around your steerer and amongst the headset spacers. The cable won't go through the stem anymore, but like mr scoble said, if you're getting a 100% custom built bike, then why would you want to change anything like the stem for reasons other than aesthetics ?
