chain line straight? the lockring problem is a weird one.. also what gear are you running? I tried 52/16 once and the chain ring would flex which would throw the chain

You haven't got a 3/32" chain on a 1/8" sprocket have you?
As for the lockring, you're saying it came loose when it was on the non-drive side? Very odd.

chain line looks straight to me (is there a way to ensure it?), and the gearing is the same as with fixed where I had no problems. The chain-sprocket thing could be it, I will look into it. It could be that the chain was worn and had stretched a bit, meaning it comes off the new sprocket easily?
The lockring thing is bugging me though!

tis very strange, if it works fine when riding fixed you would think it would be fine running ss, so maybe a worn chain issue, does it throw off at the front or the back? im guessing the back.
Im still puzzled by the lock ring haha!

If you don't skid much then the lock ring could have been loose without you realising... when you flipped the wheel, the inertia was working the lockring loose rather than keeping it in place.

Jono, that sounds very probable, I had a back brake & couple of operations on my knees in the past, so rarely skidded before

possibly is a 1/8" chain on a 3/32" freewheel sprocket with a chainline that's a bit off. 1/8" chains tend to skip off 3/32" sprockets in my experience, esp if the chainline's off, even by a few mm.

So I got a wheel a while back, fixed one side, freehub (I think it's called!?) the other.
I had only used the fixed side so far, which I am pretty sure I attached properly, and hadn't had too many problems.
However I had had problems with my knee, so I decided to buy a shimano ss freewheel and try out the other side... and this morning I screwed it on to the freehub side of the wheel, put the chain back on and off I went to work.
However the chain must have slipped off 5 times at least on the way, I tried different tensions, made sure the wheel was bolted on as tight as possible, but it kept slipping off. Quite annoying and made me glad I had 2 brakes set up...
I also noticed that the first time it fell off, the lock ring had come off the sprocked on the fixed side of the bike...this to me seems very strange because i had never had any problems with it when I was riding fixed, it always stayed on...
Anyone have any ideas why this might have happened? Is it a common problem and am I missing something obvious?
