Sounds like a bizarre crash, and yes, the woman was probably completely stressed and couldn't think (and probably shouldn't have got in her car that morning by the sound of it).
That helmet thing--you can just see how it functions in people's minds--it's both a token of how dangerous cycling is, how cyclists put themselves in harm's way (and therefore a symbol of people's excuses not to cycle and to continue to cause themselves inconvenience by driving), but conversely how they can also be blamed even in this sort of crash, where a helmet obviously didn't even get a chance to collapse catastrophically, for not wearing one.
