What is there to stop them coming out with the axle?

You upgrading your Miche bearings?

the axle doesn't have the slightly raised section with which to push out the bearing. There's a thread about it here
http://www.londonfgss.com/thread3808.html
I'm replacing them cos they're starting to make noises (although strangely they've gone away now and I've ridden through 2-3 weeks of rain without the noise coming back).

Go to the nearest DIY shop and buy an expander / empansion bolt bolt in the right size.
Release the nut, stick the expander part into the bearing (axle removed), fasten the nut, hit against the backside of the bolt to drive the bearing out.
Used to do this with Campag hub and pedal bearings (cups). With Dura Ace hubs too.
Worked fantastically.
Use the old cup to hammer the new cup back in place (and a hammer of course). No expensive tools needed.

Cool. I knew there was a DIY method of some kind. Might even have an expansion bolt somewhere...
30 mins after posting, a solution arrives.

Looking for one of these but can't find anywhere that sells them in the UK. This is to remove bearings from Miche hubs, which can't be pushed out with the axle. There's one made by Enduro, but only a US seller:

It looks like this
Or has anyone made one? Doesn't look too complicated.
