To be fair to 14, that may not be their fault. If the person who put the lockring and sprocket on before hadn't greased it well, then removing those could have stripped the threads a little and left it in a bad way for the new stuff to be put on. There's loads of things that can surface as an issue long after the actual mistake was done.
