Have you looked at the tour riders bikes? ever watched a road cycling race? a track race?
Loads of people use 130-140mm stems.
Having said that, a 6' bloke riding a 54cm will usually throw up problems, you can always use long seatposts, but the TT length is the important one, if a whacking long stem still makes it too short then you're stuffed. Extending it by using a layback seatpost will only work if the knee-over-pedal spindle measurement isn't thrown out of the riders comfort zone.
