my mum did that when she was taking call from patient, she hear the background noise and ask them;
"are you driving at the moment?"
"oh yes I am"
which the my mum got little annoyed by how candid the patient said that
"call me back when you're not driving"
and then promptly hang up, the people on the other end should do that too.
