Cleat fitting? Bike fit?
Plenty of information on here about how to sort that out. Cyclefit or the Bike Whisperer.

The talocrural joint has many ligaments running through and around it, but without getting hands on to see your range of movement - this forum may only help to submise your injury - which is what I'm about to do...
Pain or discomfort in the area you describe, during plantar flexion, could be due to an irritation to the anterior talofibular ligament - which is the first ligament to show signs of injury, as it holds the joint stable during forced inversion (a typical fatigued cycling stroke).
Firstly - I'd advise a little bit of icing, whilst laying off the bike for a day. Whilst you're off the bike, consider the angle at which your foot is sat in your cleats - or within your clips (depending on your set up). This should help to neutralise the ankle position whilst pedalling....
And that is all i have to say about that..... ;) -

Could be a few things, fistly do you ride in shoes that are very tight at the top of the foot? ie clipless roadshoes that have a top ratchet, if so is this press on this area?
Secondly how long has it being going on? If its been going a while then it could be even be something called Anterior Impingement Snydrome of the Ankle. Or if this is the first time you have been riding long distances as others say it could well be a fit issue. This could be aggrivated by the fact that one leg might be longer then the other (very commonly the case) and hence the forces become uneven, again this is helped with a bike fit and if this is not enough special foot beds can be got.
All in all though if its an acute thing that has just come on and hurts now every time you use its prob some soft tissue damage (drunken injury, ankle twisted or even very tight shoes.) If this is the case then give yourself 2 whole weeks completely off the bike (if you can!), ice it for 10mins at a time if swollen and take some ibprofen to reduce the swelling if any. Then after two weeks lightly increase load (very very slowly) over the course of 2 weeks. If this doesnt work then its off to the GP/physio for you.
Again dont know the time frame but if you have had this for ages then got to a Gp/physio now.
Hope that helps

Its the topside of the ankle (opposite side of the heal) and its always the right side (the stronger leg)
Hurts in the center of the right angel your foot makes with your shin, on every downward pedal strike
It sounds like a ligament bothering you, you probably sprained your ankle at least once in your life, likely more than once. That is the most common ligament to sprain, it is the anterior talofibular.
The other possibility is tendonitis, probably that of tibialis anterior. I'd need to watch you pedal to see how you move your foot/ankle through your stroke.
I can recommend a course of treatment, PM me if you'd like to speak about having massage done.

Thanks for all the free unqualified medical advice.
I've had a good feel of what exactly hurts where.
Its the tendon that runs on the top of right side of my right foot and up along side the large knobly bone in the ankle (thats the proper medical term for it right?)
Looking at the diagram its definately got to be a problem with the peroneus tertius
Normally id just ingore it - but im going to Mont Ventoux and got a Triathalon all this month.

Well, that wasn't unqualified medical advice I gave you, bro, I'm a massage therapist with a degree and 5 years' experience behind me!
P.S. Many people don't have a peronius tertius. That knobly bone "in" the ankle is the head of your fibula. Try turning your foot out and up and see if you get the pain. Also, see if you can poke at the end point of where the diagram shows peronius tertius. If you get the pain, then you potentially have one (it being very close to the end point for a muscle everyone does have)! Cue excitement and clapping.

Ha Ha.
My foot that is - and im not sure why and it always happens when I do alot of riding.
I had a long ride at the weekend and sure enough my foot is hurting again.
Its the topside of the ankle (opposite side of the heal) and its always the right side (the stronger leg)
Hurts in the center of the right angel your foot makes with your shin, on every downward pedal strike
Does anyone know what the name of this debilitating illness is and if there is already a official registered charity I can appeal too for funding a recovery?
