I'm no expert, but I would say probably a poorly tuned derailleur and possibly shit chain rings.

Check derailleur first, then check wear on cassette and chain rings, then when both of these are ruled out or replaced, get yourself a new chain to.

ALso check indexing on front shifter, and that the chainrings aren't bent.

Last time I checked, your Urban Fixie hiphop skid whip only has one gear, make sure the chain goes around the chainring and the cog - then you'll be fine. One of the more convincing arguments for riding fixed or SS is that now the cool kids dont have to mess with derailleurs, thereby letting em use more brains on art projects and bands.
It does sound as if you're using the wrong combo of chain and chainring, maybe like a 10 speed chain on a wider setup, with shitty indexing.
This has recently started happening and hoping there is an easy fix...
I am riding along, shift (front) gears and suddenly chain stops turning. Stop and get off, discover a bit of the chain is stick between chain wheels. It takes some reversing of the back wheel to get the chain unstuck.
What needs adjusting or fixing? Is it a problem with the chain or front derailleur?
