two discs of wood cut to the internal diameter of the headset (for centering),
two larger discs of wood outside of these (for pressing),
two washers outside of these, and then
two nuts.
All of this is threaded onto a threaded bar, and costs a couple £ in parts.
The second pic is the tool in use, pressing a pair of headset cups (gold) into a headtube (green).
To remove the old cups just use a hammer and a flat head screw driver (I would wrap a rag around the screw driver to prevent stratching the headtube), and tap them out bit by bit. As people have said you can just bang the cups in with a hammer and piece of wood, but I have'nt tried this myself.
