You need a 1/8 chain to fit that cog..
Headset - can't really diagnose, but generally if it was OK, but now is loose, a spacer is required. Is the 'stack height' of the new stem less than the old one?
(stack height is the vertical height of steerer which the stem covers.) -

(stack height is the vertical height of steerer which the stem covers.)The only other thing is to check your technique for doing up the headset. Sorry if this sounds patronising but are you loosening the stem before you do up the star bolt?

What stem was fitted previously?
The logo has more stack than most.
Looking at the age of most of the bike it might be a 1" steerer, which will need a shim rather than spacers.

it was an ITM CX2 Bar and stem combination

You'll need a 1/8 chainring too if it fits fine on there and not on the cog (if you're replacing your chain instead of the cog)

So realistically it would be worth just replacing the cog, rather then the chain and chainring?

But chain costs less than an eai cog.

yes you could, but as h20 just said an 1/8th chain will run fine on a 3/32 chainring....
If you have an 1/8th chain you'll have more options in the future as it will run in 3/32 sprocket or chairings....if you go for 3/32 (your current charinring and sprocket) you'll never be able to run anything 1/8th.

So a 1/8th chain would be the way to go...
Im not sure on the gearing i want to run atm, as i havent decided if i want this as a commuter of race/tt bike yet. Ive recently had a large crash on my geared roadbike, so may use this with a lower gear for commuting. -

This sounds right to me.
Have made this mistake myself in the past. Just kept tightening, removing bolt, greasing, and retightening, took ages for the penny to drop.
Following on from my thread for help yesterday;
I think i have the incorrect chain for my bike after repacing the rear cog with a euroasia 15t (on da hubs). Also headset is lose after a new stem was installed and it doesnt want to tighten, will this need spacers?
