I had toe-overlap on my old bike, and this was a 58cm frame.
This is the issue that is stopping my girlfriend from riding the bike she built- whenever she gets slightly more confident she kicks the wheel again, and that confidence goes out of the window.
She said the other day that she would ride her bike "if it did not terrify me".
Solutions (apart from HTFU please) on the back of a postcard!
Might be worth riding on a track, or in parks where sharp turning isnt necessary, for confidence-building?
Or frame/forks changes as detailed above. I think Sano and Julio are right though, you get very used to it with regular riding, and steer accordingly when pedals might strike to counter it. Just need to build the confidence/ expectation of it into how you manoevre in tight spots. It won't make her fall off, since the wheel isn't going to be dragging the toes downwards if it does strike.
