in my experience if the tapers have been compromised (read no longer square) this occurs. I began having that problem once because I hadn't tightened the arm enough originally. Resulted in slight rounding of my square taper. Solution : gently filed the taper square again, secured carefully with locktite.
Never broken the arm tho.. -

But the problem was that it was always "loose"...
that is indeed your problem!
The cranks should become well secured with ONE tightening of the crankbolts. If they dont, they are fucked and any further tightening and riding with loose cranks will all damage the arm further.
I broke my left Campagnolo Athena crankarm the other day.
I think I tightened it to much. But the problem was that it was always "loose"...
Anyone got one of those?
