over tightening ?
overzealous with the spanner ?
stingy with a splash of oil ?
wrong thread combinations [slight differences erode the thread somewhat i.e don't mix and match cones/nuts and axles].could be any of these.

I keep stripping axle threads, where the cone & locknut come together.
Its happened 3 times in a row now on 3 different bikes. Ive serviced tons of bikes before, and this is a very new bewildering problem.
I just dont get it, like a curse or something! Anyone got any ideas?
Cant imagine that you would do, I certainly dont.
Its always on the rear wheel non-drive side.
Everything seems perfect at the time of assembly, its after a month or so that it happened on each bike.
