I have managed to crack the hour in a medium gear time-trial (46x17, so about the same as 48x18).
However, it meant averaging 117rpm (actually spinning a bit faster to make up for slowing down at turns/junctions), and was one of the more testing things I've had to go through on a bike. (uncomfortable too!)
