I have a dia-compe brs 101 clamp on my track bike, it's a bit ugly but does the job fine. Also I think freshtripe also do just the clamp-mount bit, which is useful (and cheaper) if you already have a brake to attach it to (the dia compe one I think come with both the clamp and a brake).
Mine came from njsframes.blogspot man, already fully assembled (I hear they're a bitch to fit) so I just had to whack mine on my forks and away I went (or not, in fact, my bike just sat there for various other reasons for ages to be honest). If I'd had to assemble the clampmount and brake together myself I would probably not be as positive about the experience, but if you get one that doesn't come ready assembled then I am sure there are people on here that have experience of fitting/twiddling.
